Appointments

Your scheduled appointment time has been reserved specifically for you. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation.

First Appointment

During your first visit, our objective is to get to know you better, become familiar with your past dental experiences and establish a treatment plan that will best meet your dental needs.

When you arrive, please be prepared to complete all insurance and health information forms. We ask that you fill out several forms that will get you acquainted with our office.

To help us with your chart and save you time checking in, you may download the form from this website. Please print and complete the form, and bring it to your appointment.

Also, please assist us by providing the following information:

  • Any panoramic X-ray taken within the past six months
  • If you have dental insurance, please bring your insurance card. This will enable us to give you an estimate of your costs.

Your initial appointment will last approximately one hour. In most cases, we will clean your teeth, and X-rays will be taken. Our dentist will perform a thorough exam and evaluate the diagnostic records, which help in diagnosing teeth decay, periodontal problems, jaw problems and any signs of oral cancer. After the examination, our dentist will provide a detailed summary that outlines any existing dental problems and proposed treatment. Take-home oral hygiene instructions will also be provided, along with any suggestions to help you care for your teeth.

Insurance, Billing and Payment Options

We realize everyone's financial situation is different. For this reason, it is our mission to provide you with affordable, high-quality treatment. We offer a variety of payment options designed to help you receive the dental care needed to enjoy a healthy and confident smile.

Our office is committed to helping you maximize your insurance benefits. However, because insurance policies vary, you are responsible to know your own individual policy. We can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a courtesy to our patients, we will bill your insurance company and then allow 45 days to render payment. After 60 days, you are responsible for the entire balance, paid in-full. If you have any questions, our courteous staff is always available.

We are participating providers of AETNA, BLUE CROSS BLUE SHIELD, CIGNA, DELTA DENTAL, HUMANA and METLIFE.

Our office accepts payments by cash, checks and most major credit cards. We also offer low- and no-interest financing options through CareCredit. If you have any questions about financing or payment, ask us! We will thoroughly explain your choices and work hard to accommodate your needs.
